HFASt Driving Study - Screening Questionnaire

University of Toronto, Human Factors and Applied Statistics Lab Driving Experiment Eligibility Questionnaire


You are invited to participate in a driving simulator research conducted by the Human Factors and Applied Statistics Lab (Director: Prof. Birsen Donmez) at the University of Toronto. 


The goal of this research is to make our roads safer by testing the effectiveness of a driver-aware voice-assistant system. The following questionnaire will help us assess your eligibility for the study. If you are eligible, you will be invited to our laboratory.

 

Please note that all information collected will be held in the strictest confidentiality. Personal data will be stored securely in the Human Factors and Applied Statistics Lab’s secure server at the University of Toronto. Under no circumstances will personal data be revealed to any third party, for any purpose. 

 

Please note that your personal contact information will only be used if you indicate that you want to take part in future lab studies.
